Important information about Affiliate IDs

  • Affiliate IDs requested via the University of Utah Human Resources Department have an end date. Previously the request had a maximum time period of 1 year, but this recently changed to have a maximum time period of 6 months.

  • Once an affiliate ID has reached the expiration date the person no longer has access to REDCap. When the person tries to login to REDCap they will get a message that their account has been locked. Both the affiliate and the PI will be notified by email 1 month prior to account expiration. At that time it can be renewed for another 6 months or 1 year.

University employees must start this process.

Passwords are created through CIS. Please go to https://cis.utah.edu and use the UNID as the username with a lowercase u replacing the first zero. The initial password will be the birthday in the format MMDDYY with no dashes, slashes, or spaces. If successful a password expired screen will appear. Click the link and create a permanent password. The username will not change.

If you have any issues logging in, please contact the Campus Help Desk at 801-581-4000 ext. They can reset the password to your DOB.
