Go to “Online Designer” on the project that the instrument you would like to copy is located on In the “Data collection Instruments” box, click on “Choose Action”, select “Download instrument ZIP” The ZIP file will then downloaded and saved on your computer (generally saved in Downloads) NOTE: You may need to compress the file on your desktop prior to uploading
Next, go to the “Online Designer” on the project you would like to upload the form onto Click on “Upload” at the top of the “Data Collection Instruments” page (note: project needs to be in development mode or draft mode to do this) Click “Browse” and find and open the ZIP file you just saved. After it's been selected, click on "Upload instrument ZIP" in the box on REDCap

To copy an existing form from one project and add to another project, do the following:

Select the project the existing form is in, click on “Project Setup”, and scroll to the bottom and Click on “Data Dictionary” Next, click on "Download the current Data Dictionary" Now copy all the rows that contain the fields from the form you want to add to your other project. Next, Download the data dictionary from the project that you want to add this form to. Scroll to the bottom and click on the first empty box (Outlined in black box in the picture below). Now paste the copied fields here. Save the excel data dictionary file that you added the new fields to. Now go back to REDCap and select the project you want to add the copied form to Go to "Project Setup" and click on "upload data dictionary" (like we did on step 1) Scroll to the bottom, click browse, and select the file you saved your new data dictionary in. (to do this, your project needs to be in draft mode) Click "Upload File," and approve the upload. The new form will now be added to your project.

Go to "Project Home" of the project you would like to copy and click on the "Other Functionality" tab at top Here, click "Copy this project" Next a page will show up about making a copy (looks similar box shown in image below). Change the Project Title and any other necessary changes. Next click "Request Copy". The REDCap Support team will then review and contact you to proceed.
