(note: Public Survey only works for first survey instrument on project)

1. Go to Manage Survey Participants and click on the "Public Survey Link" tab
2. Click on "Survey Access Code or QR Code", located below the "Public Survey URL" (circled in red in image below)
   
   
   
3. Then the box that contains the Survey Access Code and the QR Code will pop up

You can also access the QR code by going to an existing record.  Instructions are here:

1. Go to an existing record and open a survey instrument.
2. Click on "Survey options" and select "Survey Access Code and QR Code", located directly above the survey (circled in red below).
