If new user does not have a University of Utah ID:
- Fill out the form at: https://redcap-dev01.brisc.utah.edu/redcap/active/redcap/surveys/?s=TLqKYy
- The P.I. will be emailed a short survey to sign and submit
- An email about the new REDCap account will then be sent to the P.I. and to the person that requested the new account
- The new user can then be added as a user to a REDCap project by the project manager. Instructions on how to add a user to a project are at: https://wiki.chpc.utah.edu/pages/viewpage.action?pageId=339312657

If an affiliate account is needed, the University of Utah employee requesting the affiliate user must initiate below request process.

Renewal notices will be sent to the Affiliates requestor on file roughly 30 days before the set expiration date. At this time, the requestor may choose to renew or deactivate and the request is then sent to HR for review. Upon approval, an email notice will be sent to both affiliate and requestor.
