For anyone that has a University of Utah ID to become a REDCap user, do the following:
Go to: https://redcap.ccts.utah.edu
Sign in with your U of U ID or U of U Affiliate ID (unid) and your campus password.
Fill out and submit the form that comes up, use your primary email address associated with the U of U.
Affiliate ID users user your email address associated with your Affiliate ID.
You will then be sent an email to confirm your REDCap account.
The new user can now either create their own project or be added as a user to an existing project.

To be added to an existing REDCap project, you’ll need to contact the project manager to give you access to it. They will need to follow the direction on: Add New User To Your Own Project
If the new user does not have a University of Utah ID:
All new users must have a University of Utah ID to access REDCap and other BMIC tools. University of Utah Affiliate ID's from Human Resources must be requested.
University employees must start this process. To start the affiliate ID request process you will use the HR affiliate ID request form. (Log in with your unid and campus password)
The affiliate ID form requests these attributes to submit the form:
Last Name (required)
First Name (required)
Birthday (required)
Please enter the email address, street address and phone of the affiliate where possible.
Affiliate Type: 10025 Other HSC
Reason For Entry: Access to CTSI Applications (REDCap, OpenSpecimen)
REDCap must be included in the reason to be exempt from a background check for affiliate ID requests.
Start Date (now)
End Date (maximum of one year from now)
Notify (You can enter your email address and others in a comma separated list.)
Once you receive the new affiliate ID creation please notify the external user that their affiliate ID is ready with the instructions in the email from U of U HR.
Notify the external collaborator an affiliate ID is created
Include the instructions to log into CIS and set a password.
Once the user has been notified their ID is active they can be added to REDCap project(s). Instructions on how to add a user to a project are here: Add New User To Your Own Project
Important information about affiliate IDs
Affiliate IDs requested via the University of Utah Human Resources Department have an end date. Previously the request had a maximum time period of 1 year, but this recently changed to have a maximum time period of 6 months. On the request form there are fields for the start and end date. The start date by default is the date you complete the request. The end date by default is 6 months from this date.
Once an affiliate ID has reached the expiration date the person no longer has access to REDCap. When the person tries to login to REDCap they will get a message that their account has been locked.
If you request an affiliate ID using https://www.hr.utah.edu/forms/affiliate.php to provide a person without a University of Utah role access to REDCap as part of your group, you will need to renew the ID. It is easiest to do this BEFORE the affiliate ID has expired. Once an affiliate ID has expired, you will have to complete the request application again (although this process will be quicker than the initial request IF you include the current affiliate ID).
To extend the affiliate ID before it has expired: University employees must start this process. Use the https://www.hr.utah.edu/forms/affiliate.php form, selecting "Extend Affiliate Access" and then in the Previously issue UNID field add the affiliate ID (with first character a 0 not a u). If the affiliate ID is still active, it will complete the name and affiliate information and allow for the entry of a new exit date.

The following affiliate has been entered into our system. Please allow one business day after the start date of 02/14/2020 listed on the affiliate form, or overnight if the start date has already passed, for the uNID to become active.

Passwords are created through CIS. Please go to https://cis.utah.edu and use the UNID as the username with a lowercase u replacing the first zero. The initial password will be the birthday in the format MMDDYY with no dashes, slashes, or spaces. If successful a password expired screen will appear. Click the link and create a permanent password. The username will not change.
If you have any issues logging in, please contact the Campus Help Desk at 801-581-4000 ext. They can reset the password to your DOB.
Once you have reset your password go to redcap.ccts.utah.edu to log into the REDCap <PROJECT NAME> projects.
