For anyone that has a active University of Utah ID to become a REDCap user, do the following:

  1. Go to: https://redcap.ccts.utah.edu

  2. Sign in with your U of U ID or U of U Affiliate ID (unid) and your campus password.

  3. Fill out and submit the form that comes up, use your primary email address associated with the U of U.

    1. Affiliate ID users use your email address associated with your Affiliate ID.

  4. You will then be sent an email to confirm your REDCap account.

  5. The new user can now either create their own project or be added as a user to an existing project.

    • To be added to an existing REDCap project, you’ll need to contact the project manager to give you access to it. They will need to follow the direction on: Add New User


If the new user does not have an active University of Utah ID:

Important information about Affiliate IDs

  • Affiliate IDs requested via the University of Utah Human Resources Department have an end date. Previously the request had a maximum time period of 1 year, but this recently changed to have a maximum time period of 6 months.

  • Once an affiliate ID has reached the expiration date the person no longer has access to REDCap. When the person tries to login to REDCap they will get a message that their account has been locked. The Affiliates requestor will be notified by email 1 month prior to account expiration if the below process is followed. At this time, the requestor may request to renew their account, or request to deactivate as applicable.

All new users must have a University of Utah ID to access REDCap. University of Utah REDCap Affiliate ID's are requested from Human Resources following the below process.

University employees must start this process.

  1. To begin, populate the Affiliate Request Form to initiate the request

  2. Once you complete the form above, an Information Form is auto-sent to the Affiliate based on the Affiliate email provided

  3. After the Affiliate completes and submits their form, information is passed to HR to review

    1. NOTE: If the affiliate does not complete their form, no HR request can be initiated

  4. Once approved, the University of Utah PI & Affiliate will be notified of Affiliate’s ID and steps to get started


Passwords are created through CIS. Please go to https://cis.utah.edu and use the UNID as the username with a lowercase u replacing the first zero. The initial password will be the birthday in the format MMDDYY with no dashes, slashes, or spaces. If successful a password expired screen will appear. Click the link and create a permanent password. The username will not change.

If you have any issues logging in, please contact the Campus Help Desk at 801-581-4000 ext. They can reset the password to your DOB.

Your REDCap User name and Password are the same as your University of Utah uNID and password.  You can verify that you are using the correct username/password combination by logging into another campus application which a login is required.  If you have tried logging in with it and still can't access REDCap, do the following:

  • Wait for 35 minutes and try logging in again.  Once you have tried logging in 3 times and failed, the server will not let you in for at least half an hour.
